Please provide the following information:
I.The name, ID card number of the informant, and the address, telephone number and e-mail address that can reach the informant.
II.The name or other information sufficient to identify the identity of the person being reported.
III.Specific evidence for investigation.
The company shall follow Article 21 of the “Procedures for Ethical Management and Guidelines for Conduct” to keep the identity of the informant and the contents reported confidential, as well as promise to protect the informant from improper treatment due to his/her reporting.
